As of March 1, 2023 Dr. Lagosky will be out of network with all insurance companies and services rendered in office will be on a self pay basis.
Medicare Policy:
Dr. Valente does not participate with Medicare or major insurance plans that are linked to Medicare. He is unable to treat patients covered by Medicare, even on a self pay basis.
Also, please note if you have Medicare or Medicaid as your primary, we will not be able to treat you under your secondary insurance.
Dr. Lagosky is a non-participating Medicare provider. Meaning, she can treat Medicare patients, as self-pay only. Patients being treated on a self-pay basis, per Medicare guidelines, will sign a contract stating they will not bill Medicare.
